Please arrive ten minutes before your appointment time. Be prepared to complete registration paperwork in the office prior to your appointment. If there is any health insurance we may bill for you, please bring the insurance information with you and any co-pay that may be required. Uninsured patients receive a 25% discount on the office visit; we accept cash, check, Visa, MasterCard and Discover, payment is due at the time of your visit. If you need to cancel or reschedule, please allow 48 hour notice. As a courtesy, the office will call and remind you of your appointment the prior business day. If you are more than ten minutes late for your appointment, we may need to reschedule in order to keep the other patients on time. Our office accepts most all major insurance plans.
