Your time is valuable. Here's how to spend less of it in the waiting room:
1. If you're new to Sound Family Health, download, print, and complete your New Patient Welcome form. Bring it with you to your appointment, and give it to the receptionist when you check in. Download your New Patient Welcome form here.
If you don't understand a question on the New Patient Welcome form, leave it blank and we will be happy to assist you in completing the form at your appointment time.
2. Please arrive ten minutes before your appointment time so you can check in and relax for a moment beforehand, and we can process your patient registration information efficiently.

If you have health insurance, please bring your insurance information with you, as well as any co-payment which may be required. Uninsured patients paying at the time of the office visit receive a 25% discount; we accept cash, check, Visa, MasterCard and Discover. Payment is due at the time of your visit. If you need to cancel or reschedule, please allow 48 hour notice. As a courtesy, the office will call and remind you of your appointment the prior business day. If you are more than ten minutes late for your appointment, we may need to reschedule in order to keep the other patients on time. Our office accepts most all major insurance plans.
