About Sound Family Health and why we chose Membership...

We founded Sound Family Health in April of 2009. Our goal, and the goal of our dedicated staff of support professionals, was to provide the best quality care, and put the needs of our patients first. These commitments have continued to guide our practices and our policies. Frequently, this has meant taking that extra step, often behind the scenes, to smooth out our patient's journey through the rest of the health care system.

Over this same period of time, America's health care system has undergone some significant changes. Independent primary care providers have experienced a gradual decrease in reimbursements. At the same time, there has been an increase in the costs of unfunded service mandates and reporting. These trends are driving independent primary care providers like us out of business.

Many independent physicians have chosen to become employees or contractors in large hospital based organizations. Unfortunately, these physicians are no longer independent. The economic and corporate management interest of the large organizations often come into conflict with what the individual physician may believe to be in the best interests of the patient. As physicians, we did not want to be in this position. Because of this, we searched for an economically sustainable solution that would allow us to remain independent. We were successful.

In order to keep our commitments intact, Sound Family Health has become a members-only Relational Primary Care (RPC) practice.

Membership Plans

SFH Complete is designed to allow the providers and staff of Sound Family Health the ability to provide care above and beyond the covered benefits offered by your health insurance. Below is a list of extra benefits that are included in your SFH Complete Membership verse SFH Standard.


SFH COMPLETE - $180 every 12 months

  • Multiple medical problems can be addressed at a single office visit with your provider.

  • A 90 day supply on prescription refills (if applicable) plus a years worth of refills. You will need to be seen at least once every 12 months regarding your medications for this to apply.

  • "Same Day" appointments available. There is no guarantee of same day or next day availability.

  • Travel benefits available for international travel.

  • Treatment via secure messaging with your provider for certain problem types.

  • Access to 90/10 Nutrition "Clean Eating" website. This includes meal plans, recipes and education on healthy lifestyles.

  • Health Newsletter from your Provider.

  • Additional time at appointments to talk with your provider. Please request this when scheduling your appointment.

  • Referral scheduling assistance if necessary including appointments with specialists and radiology.

  • Completion of administrative forms without an office visit if appropriate. (FMLA, Sports Physical Forms, Jury Duty Excusals, etc.)


SFH STANDARD - $0

  • A single medical problem ONLY can be addressed at your office visit with your provider.

  • A 30 day supply on prescription refills with up to a 3 month supply of additional refills. This means you must be seen quarterly for further refills.

  • "Same Day" appointments available. There is no guarantee of same day or next day availability.

  • Treatment over the phone is NOT available (includes Lab and Imaging Follow Ups). Requires office visit or telemedicine visit.

  • Travel benefits NOT available.

  • Treatment via secure messaging is NOT available.

  • Does NOT include access to 90/10 Nutrition.

  • Does NOT include Newsletter.

  • Only includes standard appointment time allotted.

  • Referral scheduling assistance if necessary.

  • For completion of administrative forms a FEE applies, and an office visit may be needed.

COMMERCIAL INSURANCE - We accept all major commercial insurances. If you do not see it above call us to find out more. NOTE: We only accept Kaiser’s Access Plan

STATE INSURANCE - We ONLY ACCEPT United Healthcare Community Plan. If you have any other plan, you can not be seen in our office.

CASH PAY - We do accept cash payment for visits (if you do not have state insurance). Payment is due at the time of check in. For rates call our office today.
