Sound Family Health (SFH) is legally required to maintain the privacy of your health information, to provide you with a notice of our legal duties and privacy practices, and to follow the information handling practices outlined in our Privacy Notice.
We are happy to provide you with copies of your health records upon request. Examples of documents you may request include:
Medical notes and records related to specific conditions
Handwritten or dictated progress notes for specific treatment dates
Consultation reports
Operative reports
Pathology and laboratory reports
X-ray and imaging reports
Physical or occupational therapy notes
Records received from outside providers
Specialized test results, such as:
Angiography
Cardiac catheterization
Stress tests (e.g., treadmill)
Electrocardiograms (EKGs)
Electroencephalograms (EEGs)
Magnetic Resonance Imaging (MRI)
Myelograms
CT scans and other nuclear medicine imaging

To request copies of your records from Sound Family Health, please complete and return the Permission to Release Health Care Information Form. The form must be filled out in full to process your request.
Be sure to include the following information:
Patient identifiers – Full name and date of birth
Recipient information – Name of the person or organization authorized to receive your records
Details of the information to be released – Specify the type or date range of records
Purpose of the request – Reason for the release (e.g., personal use, transferring care, insurance)
Signature and date – Signed by the patient or their legally authorized representative

You have the right to request and receive a copy of your health information maintained by Sound Family Health, with certain limited exceptions. You may choose the format in which you'd like to receive your records and may also request that they be sent to any person or organization of your choosing.
Sound Family Health offers the following delivery options for medical records:

If you are making the request on behalf of a patient as an authorized representative, these options are also available to you.

We are committed to processing medical records requests in a timely manner, in accordance with Washington state law (RCW 70.02.080 and 70.02.090).
Records will be made available within 15 business days of your request.
If we are unable to provide the records within that timeframe, we will inform you of the delay and ensure the records are produced within 21 days of receiving your request.
If the requested information does not exist or cannot be located, you will be notified within 15 business days.

If you need copies of your radiology images, please contact the facility where the imaging was performed. Sound Family Health does not store actual image files as part of your medical record.
However, we do retain copies of your imaging reports, which we are happy to provide upon request.
